Requirements

  • 8+ years of experience in healthcare operations leadership roles.
  • Proven experience managing Community Health Workers, care coordinators, patient navigators, outreach teams, or similar community-based healthcare workforces.
  • Experience building and leading distributed field teams across multiple locations or markets.
  • Strong understanding of Medicaid populations, social determinants of health, and barriers impacting healthcare access.
  • Demonstrated success developing frontline leaders and improving operational performance through structured processes and accountability systems.
  • Excellent communication, coaching, stakeholder management, and change leadership skills.
  • Experience with programs such as ECM, Community Supports, CalAIM, or other Medicaid care management initiatives is preferred.
  • Background working with culturally diverse and historically underserved communities is highly valued.
  • Experience scaling healthcare operations during periods of rapid growth is a plus.
  • Bilingual Spanish/English proficiency is strongly preferred.

Responsibilities

  • Lead Community Health Worker operations across multiple markets, including workforce planning, deployment strategies, productivity management, and performance optimization.
  • Establish operational standards for home visits, community interventions, member outreach, engagement activities, and field documentation processes.
  • Build management frameworks that promote coaching, accountability, quality improvement, and professional development across CHW teams.
  • Partner closely with clinical and care delivery teams to ensure seamless integration of CHWs into multidisciplinary care models.
  • Monitor operational metrics and identify opportunities to improve member engagement, retention, care coordination, and health outcomes.
  • Support market expansion initiatives by developing scalable operational models while maintaining service quality and consistency.
  • Develop and mentor frontline leaders, creating a culture of ownership, collaboration, and continuous improvement.
  • Drive process improvements that enhance the effectiveness and scalability of community-based healthcare programs.
