Requirements
- Bachelor of Science in Pharmacy or Doctor of Pharmacy degree from an accredited college.
- Current, unrestricted pharmacist licensure in Rhode Island (or temporary 90-day license).
- Current licensure as a registered pharmacist in the Commonwealth of Massachusetts.
- Board of Pharmacy Specialties certification (e.g., BCPS, BCOP, BCCP) required within 18 months of hire.
- Successful completion of PGY-1 Residency or minimum of 3 years clinical experience.
- Proficiency with pharmacy information systems and automated dispensing technology.
- Experience with MS Word, Excel, Outlook, and web browsers.
- Effective written and verbal communication, organizational, and presentation skills.
- Ability to establish a secure, remote workspace for PHI/PII protection.
Responsibilities
- Receive and review patient medication orders remotely via the electronic health record (EHR).
- Interpret orders for therapeutic appropriateness, clinical status, and safety considerations.
- Contact providers to initiate interventions for optimized cost-effectiveness and therapeutic efficacy.
- Monitor, document, and prevent drug therapy-related problems such as contraindications and interactions.
- Provide clinical consultations to nurses and prescribers regarding drug therapy and administration.
- Perform prospective and retrospective pharmacokinetic dosing and monitoring.
- Conduct final checks for medications prepared by pharmacy technicians and verify sterile compounding formulations.
- Ensure compliance with formulary management and adverse drug event monitoring programs.
