Requirements
- Associate's Degree or higher.
- Active unrestricted clinical license (RN, SW, RD, or PharmD).
- Minimum 2 years of supervisory experience, preferably leading and developing clinical teams.
- Experience in renal care, chronic disease management, or related clinical care management.
- Proficiency with EHRs, telehealth platforms, and Microsoft tools.
- Strong ability to analyze performance metrics, patient feedback, and clinical outcomes.
- Excellent verbal and written communication skills.
- Demonstrated problem-solving, conflict-resolution, and organizational skills.
- Knowledge of healthcare regulations, including HIPAA.
- Commitment to patient confidentiality and ethical care.
- Cultural competence when working with diverse populations.
Responsibilities
- Provide leadership, coaching, and development for direct reports to maintain an engaged and productive workforce.
- Monitor, supervise, and balance the workflow of the interdisciplinary virtual clinical care team.
- Ensure adequate staffing levels and manage clinician schedules to meet patient demand.
- Partner with managers to set, communicate, and monitor clinician performance goals aligned with program objectives.
- Provide ongoing coaching, constructive feedback, and performance support to clinicians.
- Identify opportunities for improving processes and workflows to enhance efficiency and care quality.
- Conduct regular quality assurance reviews of recorded clinician calls to ensure optimal patient experience and adherence to standards.
- Assist with interviewing candidates and training of new hires.
