Requirements

  • Doctor of Pharmacy (PharmD) degree.
  • Active, unrestricted pharmacist license in at least two of these states: AL, KY, LA, MS, NC, KS, PA, UT, VA, WV, MI, DE.
  • Willingness to obtain and maintain additional state licenses within 6 months of hire.
  • Minimum of 3 years of experience in value-based care, ambulatory care, or community pharmacy settings.
  • In-depth knowledge of pharmacotherapy and pharmacy practice.
  • Strong verbal and written communication skills.
  • Proficiency in telephony platforms and various Electronic Health Record (EHR) systems.
  • Ability to work in a fast-paced environment and adapt quickly to change.
  • Ability to work between 8 AM - 7 PM EST as dictated by business needs.
  • Familiarity with Medicare, HEDIS, and STAR ratings (preferred).

Responsibilities

  • Counsel patients via telephonic outreach on medication adherence and address medication-related problems.
  • Perform focused clinical chart reviews to inform patient interventions.
  • Document patient commitment and address adherence barriers in real time.
  • Escalate complex clinical cases to prescribers or interdisciplinary teams.
  • Collaborate with physicians and staff to optimize medication management workflows.
  • Identify and address gaps in medication management through patient interactions.
  • Participate in the design and evaluation of pilot programs or process improvements.
  • Develop standardized clinical resources, educational tools, and protocols.
  • Meet and sustain performance goals such as STARs and HEDIS.
