Requirements
- High school diploma or GED.
- Strong phone communication skills and comfort speaking with diverse individuals.
- Proficiency with websites, apps, and multitasking on computer programs.
- Ability to work in a self-directed, fast-moving environment.
- Willingness to earn a Community Health Worker (CHW) certificate.
- Reliable internet connection and a quiet, private workspace.
- Must pass a drug test and background check.
- Must be able to pass an onboarding final test with a score of 80% or higher.
Responsibilities
- Call patients and ask health-related questions using a set script.
- Identify patient needs and share appropriate referrals to health and mental health services.
- Type patient responses accurately into an Electronic Health Record (EHR) system.
- Maintain daily calendar, conduct reminder calls, and meet weekly performance goals.
- Follow federal HIPAA privacy rules and billing standards during all interactions.
