Requirements
- Doctor of Physical Therapy (DPT) or Masters in Physical Therapy (MPT) degree.
- Current unrestricted Physical Therapy license in Georgia, Texas, Colorado, and/or Indiana (Compact License preferred).
- Minimum 3 years of experience in physical therapy, musculoskeletal care, care navigation, or telehealth.
- Proven track record managing patient interactions in high-volume, performance-oriented environments.
- Strong communication, motivational interviewing, and patient engagement skills.
- Demonstrated ability to utilize technology and data for workflow efficiency.
- Comfort working across multiple technology platforms, communication tools, and dual-monitor setups.
- Ability and willingness to travel up to 10% for onsite meetings and company events.
- Ability to work independently while maintaining accountability for productivity and quality standards.
Responsibilities
- Manage assigned member population from intake through program completion in a contact center environment.
- Conduct comprehensive clinical intake assessments to evaluate MSK conditions and treatment history.
- Deliver clinical navigation via telephonic and video-based interactions.
- Utilize motivational interviewing and behavioral coaching to drive member adherence and care plan goals.
- Achieve operational metrics including engagement targets, productivity, and quality documentation.
- Collaborate with interdisciplinary teams, providers, and health plan partners to ensure coordinated care.
- Document interactions, assessments, and care plans within required timelines and compliance standards.
