Requirements
- An active compact multi-state LPN/LVN license in the state where you currently reside.
- An active California state LVN license.
- At least 5 years of clinical experience as a Licensed Practical Nurse.
- Remote work readiness including stable high-speed WiFi.
- Comfortable using AI-assisted clinical tools such as Gemini or Open Evidence.
- Proven experience in patient education, coaching, and care coordination.
- Strong interpersonal communication skills for building trust with patients.
- Disciplined in clinical documentation and maintaining accurate records.
- Comfortable working independently in a fast-paced environment with evolving workflows.
- Spanish language fluency is preferred.
- Experience providing support to individuals managing chronic conditions is preferred.
Responsibilities
- Conduct outbound telephonic patient outreach and provide one-on-one coaching and support to patients managing chronic conditions.
- Help patients execute personalized care plans and achieve their goals through education focused on behavior modification, nutrition, and self-management.
- Conduct comprehensive assessments of patients' health status, lifestyle behaviors, nutritional habits, and readiness to change.
- Educate patients on disease management, medication adherence, symptom recognition, and prevention strategies.
- Monitor patients' progress and adherence through regular check-ins and remote monitoring tools.
- Document patient interactions and follow-up plans in the Cadence platform and identify barriers to care.
- Collaborate in real time with clinicians and patient support teams using tools like Google Workspace, Slack, and the Cadence Platform.
