Requirements
- An active compact multi-state LPN/LVN license in the state where you currently reside.
- An active CA state LVN license.
- At least 5 years of clinical experience as a Licensed Practical Nurse.
- Remote work readiness including stable high-speed WiFi.
- Experience providing patient education and support to individuals managing chronic conditions (preferred).
- Fluency in Spanish (preferred).
- Familiarity with AI-assisted clinical tools (such as Gemini or Open Evidence).
- Ability to work independently in a fast-paced environment with evolving workflows.
- Strong interpersonal communication skills for patient relationship building.
- Disciplined in clinical documentation and attention to detail.
Responsibilities
- Conduct outbound telephonic patient outreach and provide one-on-one coaching and support to patients managing chronic conditions.
- Help patients execute personalized care plans through education focused on behavior modification, nutrition, and self-management.
- Conduct comprehensive assessments of patients' health status, lifestyle behaviors, and readiness to change.
- Educate patients on disease management, medication adherence, and symptom recognition according to clinical policies.
- Monitor patients' progress and health outcomes through regular check-ins and remote monitoring tools.
- Document patient interactions and outcomes in the Cadence platform, identifying and addressing social barriers to care.
- Collaborate in real time with clinicians and support teams using Google Workspace, Slack, and the Cadence platform.
