Requirements
- Active compact multi-state LPN/LVN license in the state of residence.
- Active California (CA) LVN license.
- At least 5 years of clinical experience as a Licensed Practical Nurse.
- Remote work readiness with stable high-speed internet.
- Proficiency in patient education, coaching, and care coordination.
- Strong documentation skills and attention to detail for clinical records.
- Reliability in schedule adherence and interpersonal communication.
- Comfort working independently in a fast-paced, evolving remote environment.
- Familiarity with AI-assisted clinical tools (e.g., Gemini, Open Evidence) and ability to apply clinical judgment.
- Experience with chronic condition management is preferred.
- Fluency in Spanish is preferred.
Responsibilities
- Conduct outbound telephonic patient outreach and provide one-on-one coaching for chronic conditions.
- Help patients execute personalized care plans focusing on behavior modification and self-management.
- Conduct comprehensive health status and readiness-to-change assessments.
- Educate patients on disease management, medication adherence, and prevention strategies.
- Monitor patient progress, treatment adherence, and health outcomes via check-ins and remote tools.
- Document patient interactions and follow-ups in the Cadence platform.
- Collaborate in real-time with clinicians and support teams using Google Workspace, Slack, and the Cadence platform.
