Requirements
- Active California state LVN license.
- Active compact multi-state LPN/LVN license in the state where you currently reside.
- At least 5 years of clinical experience as a Licensed Practical Nurse.
- Remote work readiness with stable high-speed WiFi.
- Experience providing patient education and support for chronic conditions (preferred).
- Fluency in Spanish (preferred).
- Proficiency with AI-assisted clinical tools such as Gemini or Open Evidence (preferred).
- Ability to navigate and troubleshoot multiple systems simultaneously.
- Strong documentation and follow-through skills.
- Effective interpersonal communication skills for building trust over the phone.
Responsibilities
- Conduct outbound telephonic patient outreach and provide one-on-one coaching and support to patients managing chronic conditions.
- Help patients execute personalized care plans and achieve goals using evidence-based techniques like motivational interviewing.
- Conduct comprehensive assessments of health status, lifestyle behaviors, and readiness to change.
- Educate patients on disease management, medication adherence, and symptom recognition under clinical supervision.
- Monitor progress and health outcomes through regular check-ins and remote monitoring tools.
- Document interactions, outcomes, and follow-up plans in the Cadence platform.
- Collaborate in real time with clinical and support teams using tools like Google Workspace and Slack.
