Requirements
- Active compact multi-state LPN/LVN license in the state where you currently reside.
- Active California (CA) state LVN license.
- At least 5 years of clinical experience as a Licensed Practical Nurse.
- Remote work readiness with stable high-speed internet.
- Experience providing patient education and support to individuals with chronic conditions.
- Proficiency with AI-assisted clinical tools (e.g., Gemini, Open Evidence) and ability to articulate their use in care delivery.
- Comfort navigating multiple software systems simultaneously while maintaining patient engagement.
- Ability to work independently in a fast-paced environment with evolving workflows.
- Strong interpersonal skills to build trust with patients over the phone.
- Consistency in schedule adherence during Pacific Time business hours.
Responsibilities
- Conduct outbound telephonic patient outreach and provide one-on-one coaching for chronic conditions like type 2 diabetes and hypertension.
- Help patients execute personalized care plans focusing on behavior modification, nutrition, and self-management strategies.
- Conduct comprehensive health assessments regarding lifestyle habits and readiness to change.
- Educate patients on disease management, medication adherence, and symptom recognition protocols.
- Monitor patient progress and treatment adherence through regular check-ins and remote monitoring tools.
- Document patient interactions and follow-up plans in the Cadence platform and identify barriers to care.
- Collaborate in real-time with clinicians using Google Workspace, Slack, and the Cadence Platform.
