Requirements
- Active compact multi-state LPN/LVN license in current state of residence.
- Active CA state LVN license.
- Minimum 5 years of clinical experience as a Licensed Practical Nurse.
- Remote work readiness with stable high-speed internet.
- Experience providing patient education and support for chronic conditions (preferred).
- Fluency in Spanish (preferred).
- Proficiency with AI-assisted clinical tools (e.g., Gemini, Open Evidence).
- Strong patient education, coaching, and care coordination skills.
- Attention to detail for clinical record documentation.
- Ability to work independently in a fast-paced environment.
Responsibilities
- Conduct outbound telephonic patient outreach and provide one-on-one coaching and support to patients managing chronic conditions.
- Help patients execute personalized care plans and achieve goals focused on behavior modification, nutrition, and self-management.
- Conduct comprehensive assessments of health status, lifestyle, and readiness to change.
- Educate patients on disease management, medication adherence, and symptom recognition.
- Monitor patients' progress and adherence to treatment plans through remote monitoring tools.
- Document patient interactions, outcomes, and follow-up plans in the Cadence platform.
- Collaborate in real time with clinicians and support teams using tools like Google Workspace and Slack.
