Requirements
- Active CA state LVN license.
- Active compact multi-state LPN/LVN license in the state where you currently reside.
- At least 5 years of clinical experience as a Licensed Practical Nurse.
- Remote work readiness with stable high-speed internet.
- Proficiency in providing patient education and support for chronic conditions.
- Experience using AI-assisted clinical tools (e.g., Gemini, Open Evidence) and understanding of their role in care delivery.
- Excellent interpersonal and telephonic communication skills.
- Ability to work independently in a fast-paced, evolving environment.
- Disciplined in documentation and maintaining accurate clinical records.
- Fluency in Spanish is preferred.
Responsibilities
- Conduct outbound telephonic patient outreach and provide one-on-one coaching for chronic conditions.
- Help patients execute personalized care plans through education on behavior modification, nutrition, and self-management.
- Conduct comprehensive assessments of health status and readiness to change.
- Execute care delivery tasks in accordance with clinical policies and protocols under supervision.
- Monitor patient progress and treatment adherence through check-ins and remote monitoring tools.
- Document interactions and identify barriers to care to connect patients with necessary resources.
- Collaborate with clinicians and support teams via the Cadence Platform, Google Workspace, and Slack.
