Requirements
- Active compact multi-state LPN/LVN license in the state of residence.
- At least 5 years of clinical experience as a Licensed Practical Nurse.
- Fluency in Spanish.
- Remote work readiness including stable high-speed WiFi.
- Experience providing patient education and support to individuals managing chronic conditions.
- Proficiency with AI-assisted clinical tools (e.g., Gemini, Open Evidence).
- Strong skills in patient coaching, care coordination, and motivational interviewing.
- Ability to navigate multiple systems simultaneously while maintaining patient engagement.
- High level of discipline in clinical documentation and attention to detail.
- Active CA state LVN license is highly preferred.
Responsibilities
- Conduct outbound telephonic patient outreach and provide one-on-one coaching for chronic conditions like diabetes, hypertension, and cardiovascular disease.
- Help patients execute personalized care plans through education on behavior modification, nutrition, and self-management strategies.
- Perform comprehensive assessments of patients' health status, lifestyle habits, and readiness to change.
- Educate patients on medication adherence, symptom recognition, and disease management protocols.
- Document patient interactions, health outcomes, and follow-up plans within the Cadence platform.
- Collaborate in real-time with clinicians and support teams using tools like Google Workspace and Slack.
