Requirements
- Fluency in Spanish.
- Bachelor of Social Work (BSW), Associates degree in nursing (ADN), Doula certification, or education background.
- 3+ years of experience in a virtual patient-facing role in patient engagement, care coordination, or social services.
- Proficiency with electronic health records (EHR) systems.
- Basic technology troubleshooting skills.
- Experience collaborating with interdisciplinary teams.
- Strong organization and attention to detail.
- Ability to work independently and manage time effectively.
- Comfort researching and memorializing community resources.
- Experience working with pediatric populations and young families preferred.
Responsibilities
- Contact patients via phone or secure messaging to schedule and confirm appointments.
- Educate patients on company services and assist with necessary documentation.
- Assist patients in setting up and accessing telehealth platforms, patient portals, and other digital tools.
- Serve as a liaison between patients, clinic staff, and providers to ensure continuity of care.
- Coordinate referrals, obtain records, and facilitate information sharing between clinical teams.
- Identify social determinants of health barriers and connect patients with community resources.
- Document all patient interactions, resource referrals, and coordination efforts in the EHR.
- Ensure compliance with HIPAA and other applicable privacy regulations.
