Requirements
- Masters degree in social work, counseling, or a related field.
- Current independent license in Connecticut or eligible to obtain it within 3 months.
- Minimum 2+ years of clinical experience.
- One year of experience working within the CoCm model with excellent quality assurance.
- Proficiency in evidence-based treatment approaches (Problem-Solving Treatment, Motivational Interviewing, Cognitive Behavioral Therapy, and Behavioral Activation).
- Technical proficiency with EHR systems and ability to quickly learn new technology.
- Experience with behavioral health tools such as PHQ9, GAD7, and CSSRS.
- Capability to provide clinical care to pediatric (ages 6+) and adult patients, including in women's health settings.
Responsibilities
- Provide indirect oversight for 10-12 Collaborative Care Clinicians including performance metrics, coaching, and schedule management.
- Support pod workflows, new hire training, and address administrative dashboard reports.
- Complete weekly chart reviews for assigned clinicians.
- Manage a caseload of up to 45 patients using the Collaborative Care Model.
- Screen and assess patients using tools like PHQ9, GAD7, and Columbia Suicide Risk Assessment.
- Coordinate care with primary care providers and facilitate care plan changes.
- Document patient progress and treatment recommendations in Salesforce and EHR systems.
- Participate in regularly scheduled caseload consultations with psychiatric consultants.
