- 5+ years of experience caring for adult patients with medical illness; 1+ years of experience caring for adult outpatients with chronic cardiopulmonary conditions
Requirements
- Completion of an accredited nursing program with a minimum of an Associate's Degree.
- Current or compact unrestricted Nursing license in the state of Virginia or Pennsylvania.
- 5+ years of experience caring for adult patients with medical illness in Primary Care, Internal Medicine, Geriatric, or Urgent Care environments.
- 1+ years of experience caring for adult outpatients with chronic cardiopulmonary conditions (COPD, congestive heart failure, or coronary artery disease).
- Strong triage skills.
- Demonstrated commitment to continuing nursing education and maintenance of license.
- Reliable high-speed internet connection (at least 25 Mbps).
- Strong interest in learning and ability to work as a self-directed team player.
- Experience with telemedicine or remote care extension (preferred).
- Experience coordinating designated health services like home health or DME (preferred).
- Background in Cardiac or Pulmonary Rehabilitation programs (preferred).
- Background in exercise science (preferred).
Responsibilities
- Perform telemedicine visits to provide patient education regarding chronic conditions.
- Lead exercise sessions for patients over telehealth.
- Assess and triage adult outpatients with multiple chronic conditions, focusing on lung disease and heart failure.
- Coordinate care with primary care providers, specialists, and external care agencies.
- Advocate for patients to improve access to medications, transportation, nutrition, and housing.
- Serve as a continuous point of contact for patients and families across the care spectrum.
- Engage with interdisciplinary teams to optimize care delivery models.
