Collaborate with a dedicated team, supporting a substantial patient panel alongside Primary Care Providers (PCPs).
Triage inbound communications, including calls, faxes, email and instant messages from teammates, patients and partners.
Act as a support system and escalation point for LVN/LPNs within the team.
Engage in comprehensive chart preparation and provide crucial patient education on care plan adjustments.
Oversee post-appointment processes, including diagnostic orders, referrals, and medication orders.
Participate in daily huddles, offering guidance and fostering teamwork among your PCPs and MA teammates.
Conduct virtual patient assessments, developing personalized care plans and monitoring health outcomes.
Serve as a pivotal communication link among patients, PCPs, specialists, and other healthcare professionals.
Promote patient health through education on preventive care and self-management strategies.
Participate in crisis intervention and palliative care discussions.
