Experience: 2–3 years

Requirements

Active, unrestricted Registered Nurse (RN) license in a compact state.
Ability and willingness to obtain additional state licenses upon hire.
Minimum 2–3 years of clinical nursing experience.
Prior experience providing care in a telehealth or virtual care setting.
Demonstrated experience in Chronic Care Management (CCM), Remote Patient Monitoring (RPM), or primary care coordination.
Proficiency with EHR systems and telehealth platforms.
Strong clinical assessment and care coordination skills.
Excellent verbal and written communication skills.
High level of organization, attention to detail, and time management.
Ability to work independently in a remote environment.
Dedicated, private home workspace.
Technical requirements: Computer (Windows 10+ or MacOS 13+), wired headphones, wired internet connection (50 Mbps down / 20 Mbps up), and webcam.

Responsibilities

Prepare comprehensive Annual Wellness Visit (AWV) charts to support provider efficiency and CMS compliance.
Conduct face-to-face virtual reviews with patients via Zoom to summarize health histories, medications, and preventive screenings.
Develop, implement, and monitor individualized care plans for Chronic Care Management (CCM), Remote Patient Monitoring (RPM), Remote Therapeutic Monitoring (RTM), and Advanced Primary Care Management (APCM).
Identify and close care gaps such as overdue screenings, labs, and follow-ups to ensure continuity of care.
Utilize motivational interviewing to engage patients and caregivers in care planning and adherence.
Communicate patient status and care plan updates to primary care providers and interdisciplinary teams.
