Requirements
- LCSW licensure in Tennessee (LPC, LCSW, LMFT)
- Experience with adult and adolescent populations
- Experience with substance abuse populations is preferred
- Background in maternal substance abuse is preferred
- Familiarity with Medicare and Medicaid procedures
- Prior remote work experience
- Ability to travel 25% of the time
Responsibilities
- Assess, plan, and implement individualized care strategies for members.
- Collaborate with members, families, and healthcare providers to develop plans of care.
- Conduct individual counseling and group therapy sessions for adolescents and adults.
- Identify and initiate referrals for social, community, and state supportive services.
- Advocate for members and families to ensure their needs and choices are represented.
- Determine appropriate levels of care using approved clinical criteria.
- Document member assessments, care plans, and referrals.
- Participate in interdisciplinary team meetings and utilization management rounds.
