Requirements
- High school diploma or GED (associate degree or higher preferred).
- Experience in healthcare coordination, patient support, coaching, or medical office roles.
- Comfort conducting structured follow-up conversations focused on motivation and behavior change.
- Strong organizational skills with reliable follow-through.
- Ability to operate within structured, evolving workflows.
- Excellent written and verbal communication skills.
- Ability to work effectively in a remote environment.
- Experience in telehealth, digital health, musculoskeletal care, or physical therapy preferred.
- Spanish language proficiency preferred.
Responsibilities
- Serve as primary point of contact for patients following PT or APP triage.
- Guide patients through recommended next steps throughout the care episode.
- Conduct proactive follow-up outreach to support adherence and re-engagement.
- Manage care navigation and transitions across levels of care.
- Execute and track referrals and external documentation.
- Use coaching techniques for goal setting, accountability, and problem-solving.
- Collaborate with care team members and escalate clinical concerns.
- Evaluate and improve care coordination workflows and policies.
- Document patient communication and outcomes in the EHR.
