Job Details

Experience: 5+ years

Requirements

5+ years practicing as an attending physician.
Medical degree from an accredited US medical school.
Successful completion of an accredited US residency program in Family Medicine or Internal Medicine.
Active, unrestricted license to practice in the US and willingness to obtain licenses in all US states.
Active certification with American Academy of Family Physicians or American Board of Internal Medicine.
Menopause Society Certified.
Experience leading collaborative, team-based care.
Exceptional organization and attention to detail.
Proficiency with multiple tech platforms and ability to adapt to new systems.

Responsibilities

Provide medical leadership and collaborative clinical support to a multi-disciplinary team.
Be clinically responsible for an assigned population of patients and population health metrics.
Provide collaborative physician supervision and consultation for APPs, RNs, and other clinical staff.
Lead multi-disciplinary team-based care coordination meetings to develop holistic care plans.
Partner with Operations and Product leads to drive population health outcomes and process improvements.
Facilitate cross-disciplinary review sessions and perform periodic peer reviews for medical providers.
Take regular virtual call to support 24/7 patient availability.
