- Work closely with clinical staff to provide care navigation support for patients who need help finding providers or connecting to social services like housing, transportation, WIC, and SNAP.
- Monitor member support phone line and address patient inquiries.
- Complete eligibility checks for new and existing patients and manage the patient offboarding process.
- Oversee patient scheduling, including prioritization, appointment reminders, and provider schedule optimization.
- Manage referrals to health plan’s Case Management team.
- Collaborate with Market Operations Lead to develop new workflows focused on improving care coordinator efficiency.
