Requirements
- Current, unrestricted LCSW license in California (BBS License required).
- Master's degree in Social Work (MSW).
- Minimum 1 year post-licensure clinical experience.
- At least 2 years working with older adults and geriatric populations.
- Strong knowledge of geriatric mental health and age-related psychological changes.
- Experience with standardized geriatric assessment tools (BIMS, PHQ-9, MoCA, MMSE).
- Understanding of California's LCSW scope of practice.
- Experience with crisis intervention and risk assessment.
- Proficiency with Epic or similar EHR platforms.
- Understanding of CMS and DHCS regulations related to PACE.
- Knowledge of Medicare and Medicaid eligibility.
- Understanding of Nursing Facility Level of Care (NFLOC) criteria.
Responsibilities
- Conduct comprehensive psychosocial assessments for prospective PACE participants.
- Perform specialized assessments for behavioral health and psychosocial referrals.
- Evaluate applicants' capacity for informed consent and decision-making.
- Assess safety of prospective participants in community-based settings.
- Complete detailed social histories including family and support systems.
- Administer and interpret standardized assessment tools like PHQ-9, GAD-7, MMSE, MoCA.
- Collaborate with the interdisciplinary team to develop preliminary care plans.
- Provide crisis intervention and safety planning when indicated.
- Document assessments and eligibility determinations per Title 22 standards.
- Serve as a liaison between participants, families, and community resources.
