Requirements
- Active, unrestricted independent clinical license in at least one U.S. state (e.g., LCSW, LICSW, LPC, LCPC, LPCC, LMHC, or LMFT)
- Bilingual in English and Spanish
- Bachelor’s degree in nursing, social work, or other health and human services disciplines; Master's degree strongly preferred
- At least two years of clinical care management experience
- Experience working with people with cancer or advanced illness
Responsibilities
- Engage in an initial clinical member biopsychosocial intake including a safety screen and administering behavioral health assessments
- Engage in follow-up visits and asynchronous interactions, including brief interventions
- Act as the key care coordinator and liaison between the referring specialist and the practice
- Lead weekly interdisciplinary team meetings with the Consulting Psychiatrist and Health Coach
- Communicate closely with the Cerula Care behavioral health care team
- Document member progress in a HIPAA-compliant electronic medical record system
- Identify patients who are not improving and report concerns to the medical provider and Consulting Psychiatrist
- Facilitate referrals for clinically indicated services outside of Cerula Care
