Responsibilities

  • Conduct outbound and inbound calls to educate patients about Advanced Care Planning.
  • Schedule ACP appointments.
  • Overcome patient concerns and objections by articulating the importance of ACP.
  • Respond to multimodal calls, texts, and emails.
  • Maintain HIPAA compliance.
  • Achieve key performance metrics including call volume and conversion rates.
