Responsibilities
- Initiate calls to eligible patients and explain benefits of care programs
- Assist the care team with patient communication
- Confirm patient eligibility and verify insurance coverage
- Maintain accurate records of patient interactions and documentation
- Communicate in a professional and empathetic manner
- Collaborate with internal teams and healthcare providers
- Meet or exceed established enrollment targets
