- At least 1 year of experience as a health coach
- Experience providing patient education and support for chronic conditions
- Remote work readiness including stable high-speed WiFi
- Fluency in Spanish strongly preferred
- Fluency with AI-assisted clinical tools such as Gemini or Open Evidence
- Ability to speak credibly about AI support in care delivery
Responsibilities
- Conduct outbound telephonic patient outreach and provide one-on-one coaching.
- Help patients execute personalized care plans and achieve goals.
- Conduct comprehensive assessments of health status, lifestyle, and readiness to change.
- Educate patients on disease management, medication adherence, and symptom recognition.
- Monitor progress and adherence through regular check-ins.
- Document patient interactions and follow-up plans in the platform.
- Collaborate with clinicians and patient support teams to improve workflows.
