Requirements
- Active NBHWC health coaching certification.
- Minimum 1 year of experience as a health coach.
- Experience providing patient education and support to individuals managing chronic conditions.
- Fluency in Spanish and English.
- Remote work readiness including stable high-speed WiFi.
- Fluency with AI-assisted clinical tools such as Gemini or Open Evidence.
- Ability to navigate and troubleshoot multiple systems simultaneously.
Responsibilities
- Conduct outbound telephonic patient outreach and one-on-one coaching.
- Help patients execute personalized care plans focusing on behavior modification, nutrition, and self-management.
- Conduct comprehensive assessments of health status and lifestyle behaviors.
- Educate patients on disease management, medication adherence, and symptom recognition.
- Monitor progress and adherence through regular check-ins and remote monitoring tools.
- Document patient interactions, outcomes, and follow-up plans in the Cadence platform.
- Collaborate in real time with clinicians and patient support teams.
