- At least 5 years of experience as a Medical Assistant is required.
Requirements
- Active Medical Assistant certification from an accredited association (AAMA, AMCA, AMT, NHA, NAHP or NCCT).
- At least 5 years of experience as a Medical Assistant.
- Fluency in Spanish.
- Fluency with AI-assisted clinical tools.
- Remote work readiness including stable high-speed WiFi.
- Experience in internal medicine, cardiology, nephrology or with the geriatric population (preferred).
- Experience providing patient education and support to individuals managing chronic conditions (preferred).
Responsibilities
- Conduct outbound telephonic patient outreach and provide one-on-one coaching.
- Help patients execute personalized care plans.
- Conduct comprehensive assessments of health status, lifestyle behaviors, and nutritional habits.
- Educate patients on disease management, medication adherence, and symptom recognition.
- Monitor patients' progress, adherence to treatment plans, and health outcomes.
- Document patient interactions, outcomes, and follow-up plans in the Cadence platform.
- Collaborate in real time with clinicians and patient support teams.
