Requirements
- Active, nationally recognized Certified Medical Assistant (CMA) or Registered Medical Assistant (RMA) certification.
- Current BLS certification.
- Bilingual fluency in English and Spanish.
- Direct, hands-on experience with CCM and/or RPM programs.
- Strong proficiency in Google Suite.
- Experience with electronic medical record (EMR) systems.
- Ability to work independently in a secure, HIPAA-compliant home office.
- Excellent verbal and written communication skills.
- Must live in either MST or PST time zones.
Responsibilities
- Conduct professional, compassionate outreach to patients via phone to introduce CCM and RPM programs.
- Evaluate patient eligibility and explain program benefits and expectations.
- Document verbal consent and interactions in EMR adhering to CMS and HIPAA guidelines.
- Collaborate with internal care teams to ensure smooth hand-offs.
- Maintain high standards of documentation for care planning and billing.
- Manage daily outreach queue independently.
