Requirements
- Active, nationally recognized Certified Medical Assistant (CMA) or Registered Medical Assistant (RMA) certification.
- Must live in either MST or PST.
- Bilingual fluency in English and Spanish.
- Current BLS certification.
- Direct, hands-on experience working with CCM and/or RPM programs.
- Strong proficiency in Google Suite and electronic medical record (EMR) systems.
- Ability to work independently in a secure, HIPAA-compliant home office.
Responsibilities
- Conduct professional, compassionate outreach to patients via phone to introduce CCM and RPM programs.
- Evaluate patient eligibility and clearly explain program benefits and expectations.
- Accurately document verbal consent and patient interactions in the EMR.
- Ensure strict compliance with CMS and HIPAA guidelines.
- Collaborate with internal care teams to ensure smooth hand-offs and consistent patient support.
- Maintain high standards of documentation for care planning and billing.
- Manage daily queue independently ensuring interactions are helpful and compliant.
