- Ability to work Sunday-Thursday schedule
Responsibilities
- Practice active listening, empathy, and solution-focused approaches to collaboratively engage with patients
- Prioritize effectively across multiple channels including SMS, EMR, phone, and Slack
- Provide emotional support, de-escalation, and education to patients
- Maintain professional documentation through various platforms
- Follow Ophelia's policies regarding confidentiality, compliance, and ethical standards
- Schedule visits with the clinical team and support visit attendance
- Perform UDS coordination and manage technology troubleshooting
