Requirements

National Certified Peer Specialist certification
Certified Medi-Cal Peer Support Specialist certification
6 months experience in peer-to-peer support services
Current state certification with minimum 40 hours of training
Permanent residency in the US 48 contiguous states
Availability Tuesday-Saturday 3:30 PM – 10:00 PM PT

Responsibilities

Providing structured Wellness Coaching sessions
Developing individualized care plans for behavioral change
Conducting risk assessments and mitigation
Screening for social, emotional, and behavioral difficulties
Moderating user-submitted content
Performing care coordination and case management
Conducting warm handoffs to other healthcare providers
Attending clinical supervision and training
