Requirements
- Doctor of Physical Therapy (DPT) or Master of Physical Therapy (MPT) required
- Current, unrestricted physical therapy license in Georgia, Texas, or Colorado
- 3+ years orthopedic experience
- Previous telephonic patient support environment experience
- Experience with inbound/outbound call systems
- Experience in motivational patient interviewing and coaching behavior change
- Experience working with a treatment team
- High level of comfort with technology, CRM, and Microsoft Office
- Strong communication and written skills
Responsibilities
- Conduct outreach to patients eligible for TailorCare and educate them on the value of the program
- Conduct intake assessments to identify patient clinical issues and needs
- Provide health coaching throughout the journey to support adherence and compliance to their treatment plan
- Provide condition education and shared decision making with patients
- Utilize motivational patient interviewing tools to address patient barriers
- Communicate effectively to encourage patient confidence and accountability
- Provide continuous updates, guidance, and triaging to ensure positive health outcomes
- Collaborate with provider teams, ancillary services and other community support programs
