Requirements
- 2 years of proven experience as a Case Management Assistant or in a similar administrative role.
- Experience with scheduling, documentation, reporting, and coordination tasks in a healthcare or social services setting.
- Bilingual – Spanish and English required (written and verbal).
- Experience working with US-based clients.
- Strong organizational skills with meticulous attention to detail.
- Proficiency in case management software, Microsoft Office, or similar tools.
- Excellent interpersonal and communication skills with a client-first mindset.
- Ability to maintain strict confidentiality and handle sensitive information.
- Ability to work both independently and collaboratively.
Responsibilities
- Organize and maintain case files, records, and service plans ensuring confidentiality.
- Schedule and coordinate appointments, referrals, and service authorizations.
- Act as a liaison between clients, families, and service providers.
- Monitor individualized service plans and track deadlines.
- Ensure compliance with state and agency-specific regulations.
- Collaborate with healthcare providers to support person-centered care.
- Prepare and submit reports on client progress and service utilization.
- Support case managers in resolving care-related concerns.
- Provide interpretation and translation support for Spanish-speaking clients.
- Provide feedback to improve workflows and client satisfaction.
