Requirements

  • 2 years of proven experience as a Case Management Assistant or in a similar administrative role.
  • Experience with scheduling, documentation, reporting, and coordination tasks in a healthcare or social services setting.
  • Bilingual – Spanish and English required (written and verbal).
  • Experience working with US-based clients.
  • Strong organizational skills with meticulous attention to detail.
  • Proficiency in case management software, Microsoft Office, or similar tools.
  • Excellent interpersonal and communication skills with a client-first mindset.
  • Ability to maintain strict confidentiality and handle sensitive information.
  • Ability to work both independently and collaboratively.

Responsibilities

  • Organize and maintain case files, records, and service plans ensuring confidentiality.
  • Schedule and coordinate appointments, referrals, and service authorizations.
  • Act as a liaison between clients, families, and service providers.
  • Monitor individualized service plans and track deadlines.
  • Ensure compliance with state and agency-specific regulations.
  • Collaborate with healthcare providers to support person-centered care.
  • Prepare and submit reports on client progress and service utilization.
  • Support case managers in resolving care-related concerns.
  • Provide interpretation and translation support for Spanish-speaking clients.
  • Provide feedback to improve workflows and client satisfaction.
