Act as a player/coach, building Care Extender skills and directly supporting members. Create service plans using member information and community resources. Guide members in developing skills for relapse prevention and risk reduction. Encourage members to develop work skills and participate in therapeutic activities. Educate members about health, abuse, violence prevention, and available resources. Monitor, evaluate, and record member progress. Build accountability communications for appointments. Interact with members, families, physicians, and providers to lower resource over-utilization. Request consultation from network specialists. Report critical incidents and quality-of-care issues. Build communication strategies to engage and build relationships with members. Assess members for risk, perform quick evaluations, and respond to safety plans. Develop community resource guides. Identify behavioral health diagnoses and ensure comprehensive service plans. Lead the Care Extender team to ensure adequate member assessment and progress. Coordinate caseloads and refer new members. Ensure team schedules meet program needs. Participate in screening, interviewing, and hiring new team members. Provide orientation and training, conduct performance reviews, and coach the team. Lead enrollment activities. Ensure the team meets Key Performance Indicators. Perform regular documentation reviews. Analyze team performance to identify gaps and create improvement plans. Collect and synthesize data highlighting member outcomes. Lead regular internal case review sessions. Partner with the billing team to review prepared claims.
