Deliver on target quota of families selecting a senior care option by generating daily qualified patient referrals. Maintain a portfolio of clients (social worker, case managers, discharge planners) in hospitals and skilled nursing centers. Develop, own, and grow book of accounts to increase referral volume. Cultivate new contacts through networking, cold calls, and service presentations. Educate families on care options and coordinate with Healthcare Senior Living Advisors. Leverage CRM and internal structure to manage pipeline. Pilot new initiatives, tests, and processes and provide feedback.
