Requirements:
Clinical licensure required (LCSW, CPC/LPC, LMFT, Licensed Psychologist PhD or PsyD, or equivalent). Licensure in a state where the program is active (WA, IN, NV, TX, TN, or LA) is preferred. If license is in a different state, adding one of the program's active states is required. 3-5 years of experience managing a team. 3-5 years of experience building and implementing programs. 3-5 years of experience delivering outcomes. Master's degree in behavioral health, social work, or health care management from an accredited university. Experience ensuring care is delivered according to applicable laws, regulations, policies, and procedures. Ability to develop, maintain, and strengthen relationships and partnerships. Track record of identifying needs and taking independent action to implement change; results-oriented. Ability to plan, organize, manage time, and prioritize multiple tasks and assignments. Excellent listening, verbal, and written communication skills. Strong ability to clearly and accurately relay information to staff, business partners, stakeholders, and clients. Ability to work cooperatively and effectively with others to set goals, resolve problems, and make decisions. Ability to provide case/care management in routine and crisis situations.
Responsibilities:
Act as a player/coach to build Care Extender skills and directly support members. Assess members for risk, conduct crisis intervention, and ensure member safety. Screen members for behavioral health, medical, and social needs. Create and guide members in developing safety and service plans. Educate members on illnesses, resources, and prevention strategies. Monitor, evaluate, and record member progress. Build accountability communications for appointments. Interact with members, families, and providers to lower resource over-utilization. Request consultation and diagnostic reports from specialists. Report critical incidents and quality-of-care issues. Develop communication strategies for team engagement with members. Coordinate development of community resource guides. Identify behavioral health diagnoses and ensure comprehensive service plans. Lead Care Extender team to ensure adequate assessment, appropriate service plans, and member progress. Coordinate caseloads and refer new members. Ensure team schedules meet program needs. Participate in screening, interviewing, and hiring of new team members. Provide orientation, training, and performance reviews for the team. Lead enrollment activities for market building or maintenance. Ensure team meets Key Performance Indicators (KPIs). Perform regular documentation reviews for quality and training. Analyze team performance to identify and address gaps. Collect and synthesize data on member outcomes. Lead internal case review sessions for quality and training. Participate in external case management meetings with health plan partners. Perform duties to maintain program-related and administrative data/reports. Partner with the billing team to review prepared claims.
