Handle inbound and outbound calls, responding within scope of practice and consulting with on-call clinicians.
Coordinate telehealth meetings between participants and clinicians.
Ensure timely care delivery and resolve basic logistical issues, escalating to management as needed.
Troubleshoot care delivery issues related to transportation, medication delivery, and hospital discharge.
Support clinical care and home health teams in managing transitions between hospitals, skilled nursing facilities, and other settings.
