Responsibilities
- Review patient caseloads and prepare necessary technology for virtual visits.
- Navigate scheduling tools to coordinate care with mobile clinical team members.
- Perform adult and pediatric wellness visits, health risk assessments, and cognitive screenings.
- Conduct medication reviews and provide health education to patients.
- Partner with clinical teams to evaluate vitals and facilitate blood draws.
- Provide comprehensive care coordination, including specialist referrals and documentation.
