Requirements
- Board Certified as a Family Nurse Practitioner (FNP).
- Active Family Nurse Practitioner license in NY, CT, ME, or NH.
- Experience performing wellness visits.
- Proficiency with laptops and mobile devices for electronic charting.
- Experience with HIPAA secure messaging apps for care coordination.
- Strong written and verbal communication skills.
- Ability to work independently and collaboratively in a team.
- Demonstrated dependability, professionalism, and problem-solving abilities.
- Commitment to patient-centered care, empathy, and compassion.
- Must pass a national background check and clinical license verification.
Responsibilities
- Review patient caseload and prepare technology for virtual visits.
- Navigate daily schedules using clinical technology platforms.
- Partner with in-home clinical team members to coordinate care.
- Conduct thorough health risk assessments, medication reviews, and cognitive screenings.
- Provide patient education regarding health and well-being.
- Evaluate vital signs and coordinate blood draws with in-home staff.
- Manage comprehensive patient care including specialist referrals and community resource coordination.
- Document findings and maintain care coordination through secure messaging apps.
