Requirements

Board Certified as a Family Nurse Practitioner.
Active Family Nurse Practitioner License in California.
Credentialed Medicaid provider for the state of California (Medi-Cal).
Experience performing wellness visits.
Technologically savvy and comfortable using laptops and mobile devices.
Experience with charting and HIPAA secure messaging apps.
Ability to work independently and as part of a team.
Pass national background check and valid clinical license search.

Responsibilities

Review daily patient caseload and prepare technology tools.
Navigate daily schedule and partner with in-home clinical team members (Sprinters).
Perform health risk assessments, medication reviews, and cognitive screenings.
Evaluate vital signs, arrange blood draws, and address specific patient needs.
Provide comprehensive care coordination including preventative care, specialist referrals, and documentation.
