- Availability to work Monday – Friday, 8 am - 5:30 pm CST
- Must be US-based
Responsibilities
- Conduct outbound calls to an assigned patient pool
- Educate patients on the benefits of the chronic care management (CCM) program
- Establish rapport and persuade patients to enroll
- Tailor messaging based on patient demographics and unique health needs
- Maintain thorough documentation in real-time during calls
