Requirements
- Graduate from an accredited School of Nursing (LPN, LVN, RN, or BSN).
- Current COMPACT license to practice as an RN, LVN, or LPN in your current state of residence.
- Minimum of two years of clinical experience in Med/Surg, Case Management, or home health care.
- Hands-on experience with Electronic Medical Records (EMR).
- Proficiency with Windows desktop, Microsoft Office 365, Teams, and Excel.
- Access to high-speed internet and a Windows or Mac desktop/laptop (no Chromebooks).
- Ability to work in a HIPAA-compliant home office environment.
- Strong verbal, written, and listening communication skills.
- Ability to demonstrate initiative, organization, time management, and problem-solving.
Responsibilities
- Perform telephonic encounters with patients monthly.
- Develop detailed care plans within care plan templates in the electronic health record.
- Provide prevention and intervention coaching for multiple chronic disease conditions.
- Maintain a patient panel of a minimum of 100 patients per calendar month.
- Complete encounters on 90 percent of assigned patient panels.
- Attend mandatory bi-monthly staff and monthly one-on-one meetings.
