- 2+ years of Medical social work experience; 1+ years experience Care Coordination and Navigation
Requirements
- Master in Social Work (MSW) from a Nationally Accredited School approved by the Council on Social Work Education.
- 2+ years of medical social work experience in a healthcare facility setting (e.g., Hospital, SNF, Home Health, Hospice).
- 1+ years of experience in Care Coordination and Navigation.
- Ability to maintain strict confidentiality and high level of professionalism.
- Ability to comply with all applicable laws and regulations.
Responsibilities
- Actively assist in the coordination of care across inpatient, outpatient, post-acute, ER, and home care settings.
- Engage with members and providers to develop unique care plans and navigate the healthcare system.
- Assess and support Social Determinants of Health (SDOH) needs and educate members on community resource programs.
- Participate in crisis intervention for members.
- Serve as a member advocate to assist with problem-solving and address barriers to care or care plan compliance.
- Communicate with members via phone or secure messaging to provide education on health conditions, medications, and procedures.
