Requirements
- Master's degree in social work
- Active state license in social work
- Minimum 3 years of experience in medical social work, case management, or care coordination
- Comfortable working remotely
- Strong ability to work in a fast-paced environment
- Comfort with new and ever-changing technologies
- Ability to multitask and handle multiple competing demands
- Bilingual in Spanish preferred
- Health insurance experience preferred
- Experience with Medicare Advantage preferred
- Experience working with the senior population preferred
Responsibilities
- Establish trust and build strong relationships with our patients telephonically and virtually
- Collaborate with an interdisciplinary team including clinicians, case managers, and community health workers
- Engage with patients to identify social barriers and coordinate services to maximize effectiveness
- Conduct screenings and create personalized action plans tailored to patient needs
- Support caregivers by providing emotional support and identifying additional resources
- Facilitate conversations about end-of-life planning and long-term care goals
- Coordinate care between the Behavioral Health Team and monitor outcomes
- Balance multiple competing priorities and build relationships with internal stakeholders
