Requirements
- 3+ years experience as a Board-Certified Adult or Family Nurse Practitioner.
- Prior experience in adult chronic care management, specifically in cardiology and heart failure.
- Completion of an accredited Nurse Practitioner program with a national certification.
- Active licensure in at least one of the following states: NC, TN, IN, OH.
- Preference for active licensure in 2-3 of the following: GA, KY, PA, VA, TX, FL, CA.
- Willingness to obtain additional state licenses as needed.
- Prior experience prescribing and managing evidence-based heart failure therapies.
- Ability to work from home in a HIPAA compliant environment.
- Proficiency with EMR systems, communication applications, and Google products.
- Strongly prefer previous telehealth experience.
Responsibilities
- Virtually evaluate patients and assist in creating care plans based on predesignated protocols.
- Prescribe and titrate medications based on evidence-based guidelines.
- Optimize guideline-recommended medical therapy for patients with heart failure.
- Respond to alerts escalated by the nursing team regarding vital sign deviations and symptoms.
- Independently manage clinical status changes within scope of practice and escalate as necessary.
- Liaise with nursing staff to coordinate medications, labs, and virtual appointments.
- Provide patient education regarding healthy behaviors and self-management.
- Drive improvements in disease control rates and hospitalization reduction.
