Requirements
- Bachelor's Degree in Nursing.
- Current, active, unrestricted Registered Professional Nurse license in a compact state.
- Minimum of 3 years experience in cardiology, heart failure management, or critical care nursing.
- Strong knowledge of Guideline-Directed Medical Therapy (GDMT).
- Experience with patient phone triage in-office or via telemedicine.
- Proficiency with multiple EMRs and Google Workspace applications.
- Comfortable with the virtual use of episodic vital sign monitoring devices.
- Ability to work from home in a HIPAA compliant environment.
- Availability for some holidays.
- Advanced certification (CCRN, CHFN, RN-BC) highly preferred.
- Fluency in English and Spanish highly preferred.
Responsibilities
- Triage patients using protocols to determine nurse-led interventions versus escalation.
- Perform virtual nursing assessments to evaluate symptoms, medication adherence, and disease progression.
- Educate patients and caregivers regarding heart failure medications, side effects, and adherence.
- Monitor vital signs and patient survey responses to identify and document changes in condition.
- Document all interactions and clinical interventions accurately.
- Answer, direct, and follow up on incoming patient calls.
- Identify urgent or emergent situations and initiate interventions as directed by the medical provider.
