Requirements
- Active CA state LVN license is required.
- Active compact multi-state LPN/LVN license in the state of residence is required.
- At least 5 years of clinical experience as a Licensed Practical Nurse.
- Remote work readiness with stable high-speed internet.
- Experience providing patient education and support for chronic conditions (preferred).
- Fluency with AI-assisted clinical tools such as Gemini or Open Evidence (preferred).
- Spanish language fluency (preferred).
- Ability to work independently in a fast-paced environment with evolving workflows.
- Tech-savvy to navigate and troubleshoot multiple systems simultaneously.
- Strong interpersonal skills to build trust with patients over the phone.
Responsibilities
- Conduct outbound telephonic patient outreach and provide one-on-one coaching for chronic conditions including type 2 diabetes, hypertension, and cardiovascular disease.
- Implement personalized care plans focused on behavior modification, nutrition, physical activity, and self-management using motivational interviewing and SMART goals.
- Perform comprehensive assessments of patient health status, lifestyle, and readiness to change.
- Educate patients on disease management, medication adherence, symptom recognition, and prevention strategies.
- Monitor patient progress, treatment plan adherence, and health outcomes via check-ins and remote monitoring tools.
- Document interactions, outcomes, and follow-up plans in the Cadence platform while identifying social needs and care barriers.
- Collaborate with clinical and support teams via Cadence Platform, Google Workspace, and Slack.
